Little League majors game, RH batter bunts up first base line. As he's headed to first, the ball kicks up and hits the bat, which is still in his hands.  To me it looks like at least one foot is out of the box, probably both.  Ball definitely hits the bat (not vice versa), and there was no intent on the part of the batter to hit the ball again.

What's the call and what are the material judgments (in/out of box, location of ball, something else)?

And related, what if a bunted ball ricochets off the plate straight back up and hits the bat a second time and goes fair?

If the batter is still in the box, like in you second situation where the ball kicks straight back up, it's a foul ball. For your original situation, if the batter is out of the box, he would be out for interference.

Which then means we have to determine when one is "out of the box" .   Is that one foot completely out of the box touching the ground?

Depends on the rules code.  I don't do LL, which is the OP, so I won't opine on the specifics.

You don't seem too sure, "foul ball". 

Intent.

Agreed....if it's not obvious that the feet had left the box and you can see that the batter did not intend to interfere then I would be calling foul. If he's clearly out of the box though then you have to call him out on INT.
