Any runner forced to advance on a base on balls may do so without liability to be put out.

However, that protection ends when the runner overruns or misses the base.

in your situation, to miss the base they had to have gone beyond the base (as you would not say this about a runner not yet to the base)

Thus, they have acquired the base by passing it but are liable to be out if appealed before touching the base.  The most common example I can think of (and it isn’t that common) is r1 stealing on ball four and over sliding the bag.  Theoretically if he rounded the bag instead, he could miss second I suppose.

In a traditional base on balls, I’m struggling to understand how or why a runner would miss the base…

If the runner isn't on a base and has reached his awarded base, just have the fielder holding the ball tag the runner.

Whether he touches the base or not, once he passes the base he can be tagged out...the ball is live on a Base on Balls (as opposed to Hit By Pitch)

You say that...but I just worked a JuCo game where a dude hit a home run and missed 2nd base.  I would have called an out on a subsequent appeal, except the defense alerted him to his mistake and he corrected it.

Good sportsmanship...why not?  I wouldn't want to lose a home run that way...especially if it was, perhaps, the only one I ever hit.  And unless it's the gold medal game of the national championship, I don't want to see someone lose a HR that way either...unless they're a total douchebag.,
