Is R1 out if he/she never leaves 1B bag on this play?

NFHS Rules.  0 outs, runner on first base.  Line Drive / Ground ball to 1B that hits the ground.   R1 thinking it may be caught in the air, goes back to 1B bag.   First Baseman tags R1 while he still on the 1B bag.  Do we have an out?  He is tagged before First Baseman Steps on 1B for the force out.  I believe if First Baseman steps on 1B first, then the force is off and runner would be safe staying on 1B bag.  But what if R1 is tagged first?  I think we have an out. ( and then a Double Play if 1B steps on 1B bag)

You have it correct.

A hit, fielded by 1st baseman, tag R1, then tag the bag is a double play 

A hit, fielded by 1st baseman, tag bag, then R1 is batter out. R1 safe.

Both players must be touching the base at the same time in order for R1 to be out. R1 is only out when he is tagged when R1 and BR are both at the base simultaneously.

That's exactly what "forced" means -- R1 is Forced to leave the base, so the base is not a safe haven.

That's only true when neither runner is forced.  Not applicable to the OP

Not in this case. R1 is forced to advance and may be tagged out.

One of the three triple plays I've been part of happened like this.  Bases loaded (tying run on third), last inning...sharp hit to third base who short hops it...R3 goes back to base thinking it was caught...F5 tags R3 who was standing on the base, then touches third base, then looks up to see R1 still standing on first base...easy toss to second base for the game ending triple play.

ART. 8 . . . A runner acquires the right to the proper unoccupied base if he touches it before he is out. He is then entitled to this base until he is put out, or until he legally touches the next base while it is unoccupied or until a following runner is forced (2-24-1) to advance to the base he has occupied. A runner need not vacate his base to permit a fielder to catch a fly ball in the infield, but he may not interfere.

a. If two runners are on the same base, at the same time and both are tagged, the following runner is declared out. On a force play situation, the runner who is forced to advance shall be declared out when tagged on the base or the base to which he is forced is touched by a fielder while in possession of the ball.
