It was a regular season game - September 17, 1980.  The Orioles were in the midst of a pennant race, but there were a ton of empty seats.  

 

It appears the balk was called on the third batter of the game.  The pitcher, Mike Flanagan (who committed suicide in 2011) was called for the balk -- when he went back to the dugout, he told Weaver that he actually did balk.
