Runners on first and second, 1 out. Balk is called, pitcher delivers ball and batter singles to right field.

What happens to the batter and runners.

Also, if the batter/runner was thrown out at first base would he be out and runners return to their bases?

Penalize isn't the right word.  Sure, it sucks if batter hits a double...but if the ball is dead it's dead.   They're not getting penalized...the play just didn't happen.

It can go the other way too in OBR....R1/R2, balk, batter hits single and R2 gets thrown out at the plate.   When that happens the OC will wish he had HS rules.

Rule set matters on this one.  OBR, as long as all runners, including the BR advance one base, the balk is ignored.  NFHS, balk is enforced, runners advance 1 base and it is a" no pitch". Yes, in HS we penalized the wrong party here, most guys here believe this rule should be changed in HS.

if the BR was thrown out, IIRC, the coach would have the option for the balk or the play (though he'd have to know this option as it likely is not volunteered by the umpire) 

There's NEVER an option on a balk.

If the runner's don't all advance, enforce the balk.  In this play, R1 and R2 advance, BR returns to the plate with the same count as before the balk / pitch.

Never say never.  In OBR, there is one case where the coach can take the penalty for delivering a defaced ball (ball with no runners or balk with runners) or the result of the play.  :)
