Lets say in the batting order batter 4 bats when batter 3 should be up then batter 3 bats.  Both reach on walks.  After 1 pitch to batter 5 coach tries to appeal batting out of order.  This is not allowed because 1 pitch was thrown.  Since batter 4 would be up now but is occupying 2nd base we just have batter number 5 come to plate correct?

After 4's at-bat became legal (with the first pitch to 3), the next batter should have been 5. But 5 MYTABed and 3 batted instead (BOO). So 5 is out and the next batter is 6. If appealed before 3 completed his at-bat, 5 would have taken 3's place at the plate and count.

The first pitch to B3 legitimized B4's at-bat, so the new proper batter is B5.  B3 is an improper batter.  If this is determined during his at-bat, then B5 replaces B3 and assumes his count.  Once B3 completed his time at bat, and the appeal is made prior to a pitch or play, then the proper batter (B5) is called out, R3 is removed from the bases and the other runner (the original B4) is put back on first.  B6 is now the proper batter.
