I found a copy of 'Maximinzing the Two-Umpire System' on Amazon for about $240.  I sent the seller this email:

 

Just out of curiosity, why is this book so expensive? I'm looking to get one but I can get a new one for around $70. I can't imagine anyone spending that price for this book.
