do the managers not signal 2 with the fingers like I have seen on TV before? That way the umpire knows that there is a double switch about to occur and not the possibility of multiple singular individual switches taking place.

got to work on making the rule take under 20 minutes, unless you have the old George Brett charging out of the dugout scenario that held things up. baseball being reactive in their rules, rather than proactive. like letting Griffith save money on dirtied baseballs and having the "incident" first and then overruling the incident, rather than changing the rule so there is not incident in the first place. Just sat there and pacified Griffith waiting for the "incident" to happen one day before changing the rule. Get rid of it to start with.

Seems strange that the PU wouldn't have noticed that the pitcher warming up didn't match what he had written down on his lineup card.  If he did notice it, would it be something he should ask the coach about, or do you let it go until someone asks?

Simple way to avoid this is to do what we did on submarines:  verbatim repeat-back.

Why didn't they notice during Robert's first meeting? Seems strange.

Seems like a rotten deal for the defense; you go and tell the umpire what you want to do, and because the umpire misunderstands or incorrectly writes down your substitution, your bullpen and outfield are blown up for the rest of the game?  Hard to believe that you could apparently get "Belisle for Rosario" when Molitor wanted "Pressly for Polanco." Bizarre situation...
