This occurred in a lower skill level and probably would never occur otherwise:

R1 begins his steal to 2nd base prior to the pitcher beginning his delivery but while in contact with the pitcher's plate.  He reaches 2nd base prior to the BR fouling off the pitch.  Does he stay at 2nd or return to 1st?

He would need to achieve 2B prior to the pitcher beginning his delivery. On a foul ball runners return to the base they occupy "TIME OF PITCH", which is defined as when the pitcher begins his motion to deliver the ball. 

So if he reached 2B as the pitch was in flight, he would be returned to 1B.
