Little League Juniors Injury Substitution

Juniors division bat 9. There were 2 injury substitutions. First one was a sub came in for a starter in the 3rd inning. The sub got one at bat and played one inning in the field when he pulled a hamstring and couldn't run any more. The starting player he came in for came back in the game for him. Is that a legal substitution since the injured player did not complete his 6 consecutive outs? Next, A starting player fouled the ball off of his face with a 2 and 2 count in the fifth inning resulting in a concussion. He and his sub had already met their minimum play requirements so does the sub simply come back in for him and inherit the count? Thanks
