Saw in Friday, May 27th's game SF vs CIN that an SF reliever was not allowed into the game since he was not on the lineup card.  OBR says the player shall still be permitted to play - is this an MLB-specific rule?  Can't find the video at the moment.

It's a Major League Rule.   You have to designate particularly pitchers because the league caps the number of pitchers you're allowed to have on the roster.   It's alluded to in the section immediately following the rule you were looking at.  There's an exception for extra innings and blowouts.
