First of all, don't second guess yourself.  You did what you did with information and experience at hand.  This is definitely not a second guess of what you did, but to me, what you described is an immediate ejection.  3-3-1-f-4 PEN should cover that. Remember, when something like this happens and you respond in an appropriate manner, you are sending a message to everyone.
