I would imagine this can work either of 2 ways for the MLBU, or the mechanics book may list one way, but allows other ways as long as the crew communicates with each other.

1. When West(1BU) goes out, Drake(PU) can come up and take the tag up at first or a play back into first if the ball is caught. If a base hit, Drake releases first base when the runner(R1) commits to 3rd. Hernandez(2BU) stays at 2nd and takes any play at second on R1. If R1 commits to 3rd, Hernandez moves over to take BR. Bellino(3BU) takes plays at third.

2. When West goes out, Hernandez can move over and cover tag up and play back into first if the ball is caught. Hernandez will release to home on a base hit if R1 commits to third. Bellino will move into 2nd for plays at 2nd base and pick up BR if R1 commits to third. Drake will rotate up to 3rd for all plays at third.

Of course right at the end, you will notice it looks like Drake was at home for this entire play(oops-on a second look, it shows him moving towards his left to the swipe tag position and then to the left hand batters box as if he started up towards 1st but got some kind of signal). Maybe Hernandez yelled out or gave a hand signal to stay at home and he (Hernandez) would take everything. Obviously, since he did not move up towards first to cover case number 1 above, it would seem he should have been moving down to 3rd for case number 2 above IMHO.

But, once again if he was signaled to stay home, maybe they used some kind of coverage where Hernandez handles both 2nd and 1st and Bellino stays at 3rd. However, Bellino came in from third and Hernandez saw him out of the corner of his eye at the last minute and gave way to Bellino. Obviously, there is a possibly something got messed up on this play IMHO.

Once again, communication is the key no matter what kind of rotation or coverage you would like to use. A silent movie just doesn't get the job done. Who cares if players or anyone else hears you loudly communicating with each other. If that's what it takes to get the job done on rotations and coverage, just do it. And yes, all umpires at all levels, do make mistakes although at every level we are suppose to start perfect and only get better.
