I know the rule changed in 2013, but if I have a runner on 1st and 3rd and my pitcher steps completely off the rubber and then fakes a throw to 3rd and the runner at 1st takes off, is that a balk?

Nope, as long as he legally steps back before making any motion to throw or feint. At that point, he's just an infielder. But be sure he legally steps off backwards.
