The only possible explanation is PU vocalized the 1st pitch as a  strike, and for whatever reason did not follow it up with the strike mechanic?  It certainly seems that way as both F1 and F2, and PU for that matter, don't react at all in any kind of surprised way to Stewart walking away.  They don't even turn to look in his direction, like "where's he going?", and no reaction from the benches adds to this argument.

 

Stewart has stated publically he heard the ump say something after pitch 1 and assumed he called it a strike.  Has Wolf commented at all?
