This one is probably not nearly as rare as 2 consecutive 3-pitch innings, in one game, by the same team, but it'll do.

Managed to see a triple play yesterday, in a 13U game.  And, the best part, it wasn't your typical youth-game triple play, where there's some massive cock-up by a runner, or the ball being over-thrown all over the field, etc.

R1, R2, 0 out (of course, 0 out).  Batter hits the ball to the right side.  The first baseman ranges to his right, fields, and throws to the shortstop covering 2B - 1 out.  Because F3 had to range so far away, the pitcher ran to cover first, and took the throw from F6 - 2 out.

Now, I'm already pretty impressed - for starters, we're 2 outs closer to being done, and for another, the way those 2 outs were made looked really great for that age group.  But they weren't finished yet.

The original R2 rounded 3B and roared home.  So F1 fired the ball to the plate.  It wasn't even that close.  I gave the punch out, and bang, the inning's over.

For the scorers in the crowd, that's a 3-6-1-2 triple play.
