In person; I thought it was a highly unlikely hypothetical solely for the interpretation manuals.

15U rec ball, OBR; R3, R1, 2 outs.  As F1 came set, R1 took off toward second.  About two and a half seconds later, F1 is still frozen in the set position and R1 is now standing on second base.  About a second later, F1 finally begins his pitch and the batter fouls it off.

R1 remains standing on second base.  To my surprise, no one said a word about it - so, either (a) the defensive coaches were not paying attention or (b) the defensive coaches actually knew the rules!

Since the defensive manager earlier in the game had to be reminded that on an U3K w/ 2 outs, the batter still has to be put out, I'm gonna go with option (a).

option (c ) - they were going to let him go anyhow so they didn't care even though they should have because it screwed up a force possibility.

I once saw an R1 steal 2nd base on a foul tip.  I correctly signaled "foul tip" with the finger tips touching each other followed by a silent strike call.  Never once did I utter "foul" or any words for that matter.  Watched R1 trot back to 1B, and no one did a thing.  Exactly the opposite of yours, but still something I thought I would never actually see!
