after 10 years umpiring (the last 3-4 relatively seriously) I had my 1st 3 pitch inning in an 11 yr old travel game. Grounder to 3rd, grounder to short and a grounder to short that the pitcher tried to screw up for me but missed.

Of course I paid for it later with 2 extra innings
