Our state association sends out a short 5 or 6 question quiz each week.

So . . . what am I missing on this question?

3.Which of the following statements is true about defensive charged conferences?
(1 Point)
 
Each team is allowed 3 defensive charged conferences during a 7-inning game before having to remove the pitcher for the rest of the game.
A coach may visit the mound twice in the 1st inning without having to remove the pitcher.
A team is not charged with a defensive conference if the coach visits the mound and makes a pitching change.
All of the above

So, I see two correct answers (A and C), one incorrect answer (B), and one impossible (D - all of the above) answer.

Since only one can be correct, I started parsing language.  I figured the first one looked like a trick . . . as if it was saying on the third conference (not on each successive one). 

Well . . .

OK, I'm going to plead a moment of complete and utter ignorance and stupidity on this one.

Why do I have it in my head that it is one per inning?  (No, it is not because of the extra innings conferences.)

I even know that I used to know this, because it is one I used to use to grill coaches on.  (Coach, you can use 'em all right now, if you want.)

I'm a moron. Reading is fundamental to understanding. My eyes saw offensive and my brain read defensive.

Thats not it.  Yes, that is correct, but no, that’s not why I was having a mental deflation moment … 

Was this a thing in the past?  Way back in the past?  
 

Now I am just intrigued as to where this notion came from.  It seems to me as a kid playing ball it was the case.  Or maybe it is just another viral umpiring infection?
