A feint has to include a step, no mater what level and what base.

 

The move was a balk in at least two ways -- but the runners only get one base.

he swings his foot toward 1B, but before it touches the ground, he twists around to throw to 3B. 

The question was asked of me, I don't know if/how he could also feint to 1B, but I've seen enough strange movement, that I've learned that anything could be possible.
