pitcher substitution

Hello !! 
By rules, we know that a pitcher (starter or reliever) must throw to the first batter he faces until this one reaches 1st or is out, etc ....is this available in a case like this: can a manager change his starting pitcher as he takes his 5 warm-up pitches, in the 6th inning for example?

Thanks to a stunt Joe Madden pulled a few years ago. MLB added:

OBR 5-10(i)  If a pitcher who is already in the game crosses the foul line on his way to take his place on the pitcher’s plate to start an inning, he shall pitch to the first batter ...

Same in NCAA.  In FED, the pitcher can be changed.

As you mentioned 5 warm-ups you must be under HS rules - so yes.

With OBR rules  - (they get 8 warm-ups) - you cannot.

Wasn't just Madden. It was somewhat common. 

Often done when their team took a big lead  in the prior half - put the starter out there to 1) stall and 2) let him get an ovation as he walked off.

Just to be clear, the number of warm-ups does not correspond  to whether crossing the foul line makes a difference in the rule codes.
